Administered by Arizona Health Care Cost Containment System (AHCCCS), as part of AHCCCS Housing Programs (AHP), the Community Living Program provides fixed site, permanent supportive housing for individuals with a severe mental illness (SMI) designation. Community Living Program units are purchased or rehabilitated by AHP on behalf of provider owner/operators using Arizona State SMI Housing Trust Funds and become subject to covenants, conditions, and use restrictions that require providers to serve persons with an SMI designation. Additionally, AHP often provides rental support or subsidies for program members residing in Community Living Program units. AHP is designed to "provide safe, high quality, economically, and programmatically sustainable housing with individualized support services to ensure stable housing for all eligible members as a foundation to improve their physical and behavioral health outcomes, well-being, and self-determination. AHCCS contracts with Arizona Behavioral Health Corp. (ABC) and HOM, Inc. to administer AHP. ABC is responsible for all system-level admin and HOM, Inc. provides direct assistance to tenants.
