Eviction Prevention and Housing Support Activities
Administered by Arizona Health Care Cost Containment System (AHCCCS), as part of AHCCCS Housing Programs (AHP), the Eviction Prevention and Housing Support Activities Program provides limited support to assist them in attaining or maintaining housing or to mitigate housing crises that could result in loss of housing or homelessness. Assistance can include payment of back rent, deposits or move in fees, or other one- time expenses such as housing move in kits (not to include furniture), reimbursement to landlords for member caused damages, and landlord recruitment efforts. Applicants must be at risk of eviction and/or homelessness, be referred by their clinical team and have a designation of Severe Mental Illness (SMI) or General Mental Health and/or Substance Use (GMHSU). All monetary awards are one-time per program participant, per state fiscal year. AHP is designed to "provide safe, high quality, economically, and programmatically sustainable housing with individualized support services to ensure stable housing for all eligible members as a foundation to improve their physical and behavioral health outcomes, well-being, and self-determination. AHCCS contracts with Arizona Behavioral Health Corp. (ABC) and HOM, Inc. to administer AHP. ABC is responsible for all system-level admin and HOM, Inc. provides direct assistance to tenants.

Required for income (general), zero, self-employed, or non-traditional income, citizenship/immigration status. May also include copy of eviction notice, copy of utility disconnect notice, copy of proposed lease, move-in cost sheet, verification from the utility company that clearly states the total deposit due, member current income verification, social security income, verification letter of employment, last two pay stubs.
