NYC Health + Hospitals, the largest municipal healthcare system in the country, announced that patients will receive one-on-one assistance to apply for housing. The support would help approximately 600 patients by connecting them with housing navigators who are members of a housing navigation team tasked with helping patients find housing opportunities, submit application materials, assess the quality of housing, and settle into their new homes. The NYC Health + Housing initiative is part of a growing movement around the country focused on connecting health and housing systems to ensure more holistic care and wellness outcomes. “Stable housing is critical to helping our unhoused patients manage their health,” said Leora Jontef, NYC Health + Hospitals assistant vice president of housing and real estate. “Today, NYC Health + Hospitals is making it easier for our patients to navigate the housing process in New York City and find a home.” Read an article about the initiative here.
